There's no particular way to do this, but I would do them in the order of the descriptors, just because it makes it easier for other people to follow. If you're happy with the points you have been awarded for a particular descriptor don't include it. You will sometimes have to repeat what you've said -but it doesn't matter. Just try and change the wording slightly. So if you have problems with preparing food because of your grip you will tell them about why it is difficult and the safety and reliability issues. When it comes to taking a shower, there may be similar but slightly different issues - so tell them again. I'm not sure why using a stick would mean you are therefore able to prepare food. The grip is completely different.

You have an obligation to inform the DVLA if you have a Severe Memory Problem.

It can be hard to assess whether or not one’s memory problem is severe or not. As it says on that link, you are not sure whether or not your problem is bad enough to need reporting discuss it with your GP. You need to be aware that whilst you have a legal (and moral) obligation to report a severe problem, reporting a minor one might result in your license being suspended or revoked, so it is something on which I would recommend you take personal guidance.

I am going to Catherine I don’t drive much anyway or far but yes it’s with my short term memory I have printed off the DVLA form and I’m awaiting GP getting back to me before I do anything. All I’m looking for Catherine is recognition from the DWP that I have a issue and they don’t just disregard it like what they have done.

It’s hilarious really.... I do get 2 points for aids any but I was trying for more because of my memory I am not safe to use the hob or the grill. I put the grill on walked outside and forgot a bout it .... another time I put something on the hob I was so exhausted I went and layed down on the bed next minute smoke alarm wakes me ..... anyway DM has decided that because THEY have decided I have no problem with my short term memory I cannot be awarded points for supervision....... my reply at present says dwp that’s ok because I’m not safe in the kitchen because my condition and medications affect my memory as confirmed by my gp I only make a sandwich nowbecause I can’t risk it I I’m unsafe if I’d I get distracted and walk away the sandwich is not going to go on fire is it . Also a case manager told me on the phone that a sandwich is classed as a simple meal .... I thought it had to be cooked with fresh ingredients?

The case manager is wrong. A sandwich is not a meal and they know it! And if you are having to live off sandwiches then you are not getting sufficient nutrition and you should be arguing you are unable to prepare a simple meal. And yes highlight what your GP has said, noting that your GP knows you and the DWP do not.

I recorded the call with the case manager because of my memory issues should I transcribe it and send it as evidence as well just to show I am correct when I say I was told by DWP a sandwich is a meal ? She also told me I had 13 moths to get my information in not that it will score me points but wrong info
