How far back will DoC look at medical evidence

Hi again Kevin,

In my case, I emailed my GP surgery, requesting copies of assessment reports, but they claimed they did not have them. I then called the hospital where I had been assessed and asked them how I could obtain copies of the reports. The hospital gave me the contact info for the health records team. I then had to complete a SAR application form. The health records team then emailed me my medical records, including the assessment reports. There was no charge for the records.

Hi Kevin. GP notes you can request at the Surgery. This should include copies of medical (hospital) letters.
Additional hospital info you have to do a request from the NHS through a portal.
If time is an issue, the GP ones they can just printout.
Hospital (portal) ones you may have to wait, so just state: ‘more in-depth info is forthcoming from the NHS’ or some-such.

Also….. Hospital notes may be under different Hospital Trusts depending on the housing of your friend (past and present). Check in case you have to do requests to each. Good Luck!
