Hi again, following on from my query as to which form I have I have another query.
I was away on holiday when the review form arrived, only getting back on Monday, and they have asked for it back by 13/10/21.
Whilst I way away I had some new bowel/bladder issues which I visited the GP about yesterday and have been referred for colonoscopy and pelvic scan.
I am wondering if I should ask for an extension until these tests are completed or submit the form explaining that I'm undergoing tests?
I know they're unlikely to have actually got to it before the tests are done so I guess I could send it in on time, and then advise them of the results when I have them?
Any thoughts much appreciated

It is up to you whether you want to ask for an extension. If you have knowledge of when the tests might take place and how quickly you are likely to get the results and it will be pertinent to your review then you might want to ask for an extension. But if you have no idea how quickly the referrals are going to come through or the results you might want to just right that you have had new difficulties and your doctor has referred you and this is an ongoing issue.

I think typing out the question helped me clarify my thoughts.
The tests, well one of them should come through quite quickly as its on a 2 week referral, but my review form states I shouldn't include any information about tests I'm going to have which is frustrating as surely the referral for the tests shows concerns about my condition?!
I'm going to start completing my paperwork and if I'm struggling due to my fatigue/brain fog I might ask for a delay anyway....

If you're struggling - ask for an extension. I always ignore the bit about telling them what is happing - because as you say - it explains the current situation. The reason why they don't want to know is that you don't have a result that proves the current impact.

Hi, just a thought don't you have to be suffering from a condition for 3 months before it can be considered ? As this seems very new might they disregard it anyway. Just a thought if it helps with your dilemma.
