ESA WCA Questionnaire was created by Mikey
I am currently receiving ESA (Support Group) and have recently received the WCA Questionnaire to complete. Would it be helpful if I included previous documentary evidence from health professionals relating to my condition/illness? I have letters and statements dating back to 2013 relating to my health - which hasn't changed since then. Should I include these as further evidence?

it is always good to include old documentary evidence along with new evidence as it shows that your condition is ongoing with no changes.

Thanks Gary.

Also, I have been informed to return my Assessment Questionnaire 7 days before the due date of return due to post being lost. Would it be a good idea to post the Questionnaire by recorded delivery? I have the return address on the envelope I received with the Questionnaire.

With regard to previous documentary evidence: I have a letter from my Doctor dated May 2016 stating: "He last had an assessment a couple of years ago when he was found unfit to work, nothing has changed since then. He still has the health problems and the same challenges in his life. The prognosis is he will not improve."
Would it help if I included a photocopy of that letter with my evidence?

There used to be a template of a specimen letter - it could be given to doctors to sign - on this site. It enabled us to provide supporting evidence. Has it been removed?
