My previous review for ESA was in 2013. I have just received another ESA50 form to complete. I would like some advise regarding including supporting evidence, i.e, GP letters, Physiotherapist Letter, personal statements from friends etc. Do I just need to include new letters since my last review or should I send copies of all supporting evidence I have from numerous GP’s over the last 25 years?

Thank you for your question.

The first thing I would ask you , did you keep a copy of your previous ESA50 form, if you did then use it as a template to complete the new form making any necessary changes that are relevant to you. Use what ever supporting letters you have for each condition, if you have lots of supporting documents for the same condition, I always send the most resent supporting document and the oldest to show that the condition is ongoing. Do that for each condition.

Make sure that you number each document and refer back to that document when filling in the form, also put your name and national Insurance number on each page 1 of 12 and use our latest guide to help you;
