Hi, Ive received the UC capability for work questionnaire. Am I correct in understanding there's a computer version - and does the computer version allow for extended answers/explanations?
In case needed, is an extension to the submission date standard if requested?
There is an email address you can contact the DWP on for an electronic copy of the form but I'm afraid I've not kept a copy of the address, I'm hoping one of the other Mods or a member can supply it.
The form is in PDF format and there is a restriction of 5000 words to each comment box, but you can supply additional content as a separate document.
