There is an email address you can contact the DWP on for an electronic copy of the form but I'm afraid I've not kept a copy of the address, I'm hoping one of the other Mods or a member can supply it.
The form is in PDF format and there is a restriction of 5000 words to each comment box, but you can supply additional content as a separate document.
