ESA ir UC claim sick note

How do you complete a UC online claim if you dont have a sick note, it wont allow you to complete the form and submit without dates?

I am on ESA ir HB CT PIP, and have been told by housing my claim has been stopped due to me moving in early before my new tenancy date started, and must claim UC.

Either get a Fit Note before you make your claim or make the claim for UC without a Fit Note and then add the details of the Fit Note to your UC Journal once you have seen your GP.
