If a close personal support family member is helping with a reply to PIP do I need to tell them?

As in this reply was completed from a support person and was seen by me which I agree is true.

Do I need to tell them this on the reply?

Do I name them or explain who they are to me?

Hi WalesQ

You definitely want to tell the DWP if someone else has completed your form. If you don't, they will make assumptions about your abilities that may not be true. As long as your support person doesn't mind, you should name them and their position.
