UC113 appears blank in my Patient Access account

Good morning, I have been trying to find a definitive answer to a question about UC113 form. I have 4 significant health diagnosis, one of which is birth defect another is a serious mental health diagnosis. DWP sent the UC113 to my GP, I understand this process which ordinarily is fine and as expected.
I have viewed the UC113 in my patient access account, the form appears blank apart from my name and NHS number. Does this mean the form in my PA account is redacted or does this indicate that the form UC113 was returned to DWP blank.
I am struggling to rationalise this, I have debilitating anxiety. I can’t imagine why the form would be returned blank, it makes no sense. Considering it would have a significant negative impact on my work capability assessment. My assessment is next Wednesday at 09.00, my question is a time critical. I am absolutely terrified to discover the form in my PA account is blank, does this mean it has been returned blank. I have attempted to speak to my GP practice but all I was offered is a GP appointment next Tuesday at 2 pm. My WCA is Wednesday morning at 09.00.

As you have a serious mental health diagnosis, there is a possibility that the information has been redacted - but the fact is the only person who can tell you why the form is blank is your GP. I'm sorry that I can't ease your mind on this.

Thank you BIS.
I travel fast, I have been able to postponed my assessment until I can see my GP, appointment booked for Tuesday 15th.
I spoke to DWP on the phone and made detailed entries in my journal, I also spoke to HAAS, in this instance SERCO. The agent was helpful and understanding and went out of his way to speak his manager and then cc me in the reply, so I have an indication that the issue is not of my making.
Once discussed with GP, I will ask for a full medical report, listing conditions, treatments, medications etc. I already have several box files with Psychological and Neurological letters, diagnosis and treatment. I have supplied the result of the past 3 years NRI and Nerve conduction study consultations and reports made by Neurology consultant.
My complete psychological journey is lengthy so I generally, include the same 3 years of evidence.
I am calmer now and have reached out to The Keystone Hub, Mind and AMHT. Come what may, I have good people around me. It has been a weird week

Well done you, for fighting your fears and finding out so much information. So glad you had a sympathetic agent on the phone - that doesn't always happen. Glad they are postponing the assessment. It sounds like you are going to be well prepared. I hope that you will get the assessment recorded.

I was in a similar situation before my assessment (or rather after) I found an blank UC113 form in the NHS app, but after the telephone assessment I was awarded LCWRA ayway.

I have since discovered that the health report sent to DWP by my GP practice was basically a summery from the EMS/Patient Access app.
To say I am incensed is an understatement, the top of the form says Current diagnosis affecting ability to work. For me it says Diabetes. This is disturbing to me and has had a terrible impact on me.
To explain, I have a diagnosis for BPD, Tethered Spinal Cord Syndrome, Lumbar spine degeneration on 4 levels, Li to L4 inclusive Cervical spine degeneration on 2 levels. Moderate nerve impingement at L2/L3. Meralgia Paresthetica in both legs, clonus left ankle (involuntary twitch) Ulna neuropathy in both arms and a moderate nerve impingement at C6/C7.
To say it is only my diabetes that effects my abilities is insulting and I feel unrepresented by my GP practice. Reading the report further, there are entries where my doctor has talked about my psychosis and derealisation. There are other issues I am functionally deaf for one, in its self not so bad, I have hearing aids ! A day being is not fun to be honest, I have tested my mortality a few times. None of which is even mentioned.
To say I feel vulnerable is barely a scratch. I have managed to postpone my WCA, and that has removed the urgency to a degree. I also have a GP appointment on Tuesday next week, where I will discuss this. I fear the damage is already done, an undoing that damage with be difficult. The DWP is infamous for its empathy after all. Terrified
