ESA Permitted Work & Can't Find ESA40

Hi,

I get ESA and today I received a letter asking what permitted work I'm doing. About 6yrs ago I did some permitted work (within the allowed limits). I sent a form to the DWP letting them know and carried on. The work wasn't sustainable and after lots of stops and starts, I stopped altogether a few years later. I've checked my records today and it looks like I never received a reply from the DWP regarding the permitted work form I sent in. Today's letter is the first I've heard about it.

I need to fill in the form they've sent but the options on the form are very limited: yes, I'm currently working (add more info), or, no, I'm not currently working (no more info required). I'm not currently working, but I was then. Should I include extra evidence to explain the circumstances?

Additionally, it says to complete an ESA40 if there's been a change in circumstances. I do have a change to report (an additional diagnosis that has added another barrier to work), but the ESA40 form has been withdrawn and I can't find a copy online. I've looked in the resources section on here and didn't see one there either. Will I have to phone them and ask for one?

I received a form that sounds much like the one you have received years ago after having been bullied into trying permitted work for a few hours per week. I was in the WRAG group at the time. I only did the permitted work for a short time, finished years before the form arrived.
I sent it back saying I no longer did the work and enclosed a letter from the person who I was doing the work for to confirm this. That was the end of it.
I wonder if the change of circumstances they are asking you to report is work rather than health related perhaps.
