medical questionairre for doctors for dla was created by sandra hedges
Can anyone advise what type of questions are sent out to doctors for information on their patient for dla claim. I am asking as I have completed application 11 weeks ago for change of circumstance from middle rate of care to highest rate and my doctor has the form to complete and return to them and i just wondered what type of questions are asked. :)

I would be interested in the replies to this question as I am in exactly the same situation. I have been on middle rate for about 12 months and completed an application for change of circumstances and I have been told they have sent a request to my doctor for a report.

The actual form that is sent to a claimant's GP, is not as far as I know, available online.

There is however a form which is completed by a examining medical practitioner (EMP) for DLA claims entitled "Disability Living Allowance medical report form" and it would be likely that the questions will be similar as those addressed to a GP.

The form can be accessed by scrolling down to the appropriate document in the DLA resources section HERE.
