From our research, making a new application for ESA while you wait for your appeal, appears to be the best of the choices available.
As to why you have received the form, whilst I joked about it before, there are DWP employees who try and help claimants, and I suspect it is one of those who have sent the claim forms to you.

Hi Bill. I was refude ESA on the grounds i hadn't paid enough National Insurance Contributions for my Tax year 2008/2009. I live with my Husband

If someone is in receipt Of IB which is contribution based, but fail the PCA, aren't they entitled to apply for contribution based ESA? Have wondered about this before.

Thats the understanding i thought Catherine.But like i say i have been refused ESA & recieved this form to fill in. Dont understand as they say i am not entitled to any benefits, so why do i have to fill in this Form. In fact guess what, i recieved another form exatley the same one as stated before ,in the post this morning.So now i have 2. I am even more confused
