ESA TIER 3 INDEPENDENT PANEL REVIEW - WHERE NEXT?

After a psychotic breakdown 20 years ago followed by 3 emergency referrals, admissions and sectioned/encascerated in Mental Health Units and an attempted suicide the Atos Healthcare diagnosed "mild depression" and declared "Fit for Work" at my WCA in Jan 2010.
After 12 months, a fourth emergency referral, admission and section/encasceration in a Mental Health Unit my Atos Healthcare complaint has reached Tier 3 Independent Medical Panel
Their "report" is unsigned, undated and full of basic discrepancies - even my medication has been wrongly recorded!!
Where do I go next - Independent Case Examiner/Ombudsman??

forgetting the stuff that happened 20 years ago was the recent section done under section 2 or 3 ... and you was not there at your own request ?

have you still if under 2 or 3 still got your copy of the section paper work ? as that could be used as evidance and if they under the 1983 mental health act made you have treatment that too would help your case

section 4 sectioned would IMO not carry as much weight but still could be used as evidance
