No it's not the old form as this section was not on it either, so as you thought it must be for claimants not resident in the UK, unfortunately, I cannot find a copy of the form online to refer to, so you are going to have to take it at face value.
Are you able to contact the DWP by phone, as I think you need to ask them how to handle the situation, if as you say, no doctor in Sweden is going to complete it?
If you do get a decision on what to do, follow it up with a confirmation letter.
Let us know what they say.

That sounds liek a good idea original dave, only problem is it takes a couple of weeks to see gp then up to 90 days to see a consultant which takes me well past 20th June.
Maybe I should ask for a deferral until I have time to get the report, do you think the DWP would be agreeable?

yes I have number for international DWP so will phone tomorrow morning and see what they say. I will follow up with email so it is in writing, will that be ok or is a letter better?

Well I phoned DWP and they said if the doctor wont fill it in its ok but more than likely means I will get a medical.
I am going to ask my osteopath to write a report as at least he can verify my knackered joints, lack of mobility and tense muscles. Better than nothing, well I hope so anyway!

Hello Isobel sorry to hear about the difficulties you are experiencing trying to get your GP to fill in a section of your ESA N 50. Obviouisly if you were able to have some input from your GP/specialist backing up your submission, then if you are summoned for a medical before an Atos robot, after he/she duly dishes out 0 points, I would think it would be very beneficial if you chose to appeal the decision, if the robot has deemed you fit for work, and your own GP/specialist had backed up what you had written on your form. If this situation arises then I guess a report from your osteopath would go a way in discrediting the Atos doctor at appeal stage.
I don't know if you are aware that if you have to have a medical and are living outside the UK, you have the choice of having the medical in your own country, in your instance Sweden or returning to the UK, at your own expense. I gleemed this information from a link somebody provided on this site, but I am unable to find the thread in was in. I think it was in a thread on the Discussion forum.
As myself and my wife live in France and my wife is in receipt of IB, she has decided if a medical is necessary then she will have a French doctor carry it out, thinking he or she cannot be any worse than an Atos doctor, although I think Atos are a French company, so I guess it will be the French version she will experience.
Just as a matter of interest is your reassessment date actually due now for your IB, or have they brought forward your date?
