Has anyone been awarded ESA without having to take a medical?

What sort of medical conditions would not have to have a medical. My friend has a degenerative neurological disorder and has been advised to seek ESA. However, I do not think he would be able to handle the stress of medicals etc. He was recently awarded Disability Allowance at high rate for both mobility and care but this was without a medical.
