1. I Have a letter dated june 1998 from an Adjudication officer whom looked at my appeal to up my husbands care support and won this appeal, It also mentions the Doctors report from his visit [Independant Doctor sent from them] to see my husband, This Adjudication officer letter comes from the same place I Have to send the ESA Form back to !!!
Should I Make a copy and send it as evidence back with the ESA Form ?
2. I Am filling out the form for my Husband, I Have filled out the one I Downloaded from yourselves, Would it be best to Rewrite it out on the form they sent me or just print off the Online one ?

Ok, Only a few questions left before I Post it off, My Husband [Mental health part of form, O.C.D. Anxiety, Depression] is going from Income support to ESA, He has been on DLA Higher rate Personal care for life from 2000 And Lower rate mobility for life from 1997...

Re : question 1, any evidence that supports your husbands limitaions with respect to the ESA descriptors will help, even though DWP/ATOS may well say it is a report re DLA, and is out of date.It will certainly do no harm to send copy of this.
RE : question 2.
There is no problem whatsoever with printing off and sending your typed copy of the ESA50.
The advantage is you have a ready made exact copy which can be used for future reference and even edited and reused, and you can save a back up copy to USB, or hard drive.

You've probably done this, but... Do make sure that it's stated clearly on the form that it has been filled in by someone other than the claimant (so assumptions aren't made about his abilities).2. I Am filling out the form for my Husband.
