Supersession

I'm afraid we don't have a template you can use, not even for DLA or IB so you are going to have to create it from scratch.

You might start of by stating what you are asking for and why and then go onto to explain why.

As you are currently in the WRAG, and would be asking to be placed in the Suuport Group, I would reference the SG criteria that you now believe you now qualify for and and how your mobility and the FMS affect you and how, if it is, different from when you previously filled in the ESA50.

It might help you to go through the relevant ESA guides in the Members Only area, so that you phrase your statements in a similar manner to how you would on an ESA50.
