Hi V,

Derek is correct, in that you should request all documentation, rather than specifying certain ones pertaining to your ESA decision, when submitting a SAR.

These documents should definately include your ESA50, ESA85 or ESA85A, whichever is applicable, almost certainly The LT54 (DMACR) Part 3

An ESA113 if one was sent to your G.P. for completion, and maybe a MED3 (fit note).
