Hi TT,
It is highly likely that a decision has been made by The DWP, and you have been placed into The WRAG or The SG, without the need for a face to face assessment.
It is also likely that your G.P. received an ESA113 Questionnaire at the end of August, as quoted.
You should have received a written decision notice informing you which group you had been placed into, and giving you the right to appeal the decision if you wished to.
I would advise that you contact the DWP at the address of the Benefits Office on any letters that you have.
You could also phone the 0845 number on any such letters, they should have the information on their computer screen stating which group you are in, and the recommended length of the award before you are reassessed.

SG is the Support group of ESA, the other one being the Work Related Activity Group.
Nothing on this board constitutes legal advice - always consult a professional about specific problems

Just to add,
It is important that you contact the DWP, even though it appears that you have not had to take part in any Work Related Activity(WRA) it is still possible that you have been placed into The WRAG.
If you have, and you are entitled to Contribution Based ESA (CB ESA) only, then payment will cease 365 days after the date of the award decision.
12 Month Limit for ESA(CB)
Further, if it was a WRAG award, and you wanted to appeal it, the maximum period allowed for a late appeal is 13 months after the decision.
It would seem that you are approaching that 13 months limit.
