Thanks for your reply. I dread to think how long it is going to take me to complete the PIP form if I have to start from scratch. I sincerely hope that they give you a reasonable amount of time to fill it in otherwise I will be sunk. Can it be downloaded so that I can get a head start at all? My efforts have found nothing on the DWP site.

You will have four weeks to complete and return the PIP2, however this includes postage time so it will actually be less, if you are struggling to complete it then you can request a two week extension from the DWP.

I'm afraid there is no downloadable version of the PIP2, each copy has a barcode that is unique to the claimant, so only the copy sent can be used, however, you could prepare your answers on your computer and either transcribe them to the PIP2 when it arrives or link to your answers and attach pages. If you do this then each page must have your full name and National Insurance Number at the top and we recommend that you number each page, something like "page X of Y".

I'm afraid I need more advice. After 14 phone calls (majority of which were directly to the medical assessment 0208 telephone number) and lots of chasing by my husband, they finally received the ESA113 from my GP on 7th July. It was originally sent to him back in February.
Unfortunately an assessment was booked by DWP on 4th August at a centre in Southend before the report arrived. The person I spoke to advised me that my file would be worked on again, and it should be processed before this date. I was to ring a few days before the date of the assessment if I hadn't heard anything from them.

Whilst all this has been going on my PIP application has been completed and an award made. This included a home based assessment by a nurse. I have been awarded the Enhanced rate for both components ongoing. Throughout I was treated with courtesy and in a supportive manner.

I phoned the 0208 number to inform them about the PIP award but couldn't get through. So I went back to ringing the 0800 enquiries number. The way I was dealt with made me feel as if I was something unpleasant that the person had trodden in. I asked if a decision had been made and was told that I had an assessment booked. I explained the situation about the ESA113 and also mentioned the PIP award. He made various statements which seemed conflicting. That everyone is seen once a year? That they had decided this time that they wanted to see me this time (I was transferred from IB to ESA without any medical or form to my GP at all), and that despite the medical report they still wanted to see me. Is there any other evidence you can supply was the final question? I have submitted 31 pieces of supporting evidence, including medical reports, consultant letters, OT requisitions etc.

I have made it clear that my GP doesn't deal with my condition, I see my consultant every six months and the GP just prescribes what he says. I have stated both on the form and verbally that the best person to contact is my Consultant but they refused. Because going out is such an ordeal I don't go and see my GP, my husband usually goes on my behalf. My GP is generally extremely unhelpful when it comes to filling forms in. In actual fact the nurse who came to do my PIP assessment has the same GP and she has found him thoroughly unhelpful too. I provided him with information about how my condition affects me as you advise.

I asked about having a home visit and was told I would need to get my GP to write a letter to that effect. Bearing in mind the time it took to get the form back that isn't going to happen. So my only option is to attend the assessment, despite just having one for PIP. I can't leave the house without significant help from my husband and attending an assessment will cause significant increases in pain, swelling etc getting there, during and then for the next few days after the assessment. I can count the number of times I've been out this year on the fingers of both hands.

I am absolutely furious about the way I have been treated, especially as I have now had the condition (CRPS) eleven and a half years. It has continued to spread to other areas of my body since the last ESA award. There is no cure, only palliative care is available. I really don't know what other information they could possibly want?

What options, if any do I have? Can I complain? If so, who to? To say this is stressful is an understatement. Can I speak to anyone else?

As to the phone call, the first thing to be aware of is you are talking to call centre staff, they only have limited information and no real ability to change what is happening with your claim, however, this in no way excuses for their attitude on the phone
