Hi Kim
1. Yes your daughter should be able to sign the form online.
2. If there are questions that you don't think your daughter would have scored for you can leave them blank. However, if you do so, you run the risk of possibly losing points that she might have got. I can't tell you exactly what to do, as I don't know your daughter, but if I had a relative who had scored nothing - I would simply answer them all.

Thank you Gordon. In, Ways to challenge a medical report, it says, Challenging the history section, where will I find this, I have looked on the original letter that PIP sent back all I can find on there is How I Made My Decision, nothing else on both original first letter and Mandatory reconsideration letter. Am I wrong in assuming that you have a copy of the assessment report? You would have had to ring and request a copy and I assume you will have used it to help write the Mandatory Reconsideration. You should find a history section on that.

Once you get the Assessment Report Kim, you will find the History section right at the beginning. Read very very carefully watching out for his/her opinions that may be designed to sway the decision maker towards minimising the problems your daughter has, as they read on through the assessment sections, but don't match your evidence. Eg 'medication for moderate pain' [when in fact it is for severe pain], subtle things like that. That example was from our daughter's btw. Best wishes, Denby

Hi BIS, I didn't realise that I should of requested the assessment form and it is too late now as this has to be with them by Monday.
