There's no automatic exemption from a medical when people on DLA(HRC) are migrated to ESA.
However, there are some exemptions from a medical for ESA, which you can find in our guide to claiming ESA. Also, not all migrations will be subject to a medical, since the DWP decision makers will look at what evidence they have and decide whether a medical is necessary. In relation to some people on IB, it will be evident from the information on file that they are eligible for ESA.

I think page 15 details the tests, with relevant points, that are used in the ESA medical for entry to the Worl Related Activity Group.
Page 25 details the criteria for entry to the Support Group.
