would i need to phone dwp because of the second esa50 form as stated above somewhere

In terms of an appeal to be placed in the Support Group, whilst you are now outside the normal one month, you can still lodge an appeal providing you provide Good Cause as to why it is late, the DWP, for ESA, are quite lax on allowing these at the moment.
In terms of the current assessment, yes, you need to contact the DWP.

Well done for getting through your first WFI.
In regard the GP's report, you should be aware that you have already been found Unfit for Work, so a report that says that you cannot work will be of little value, your GP needs to confirm that that you either meet one or more of the Support Group Descriptors or that you cannot perform Work Related Activity and why.
