Hi everyone 
I'm sorry to add a new post, but I can't seem to find my old one?
STILL havent heard back from PIP with any sort of forms? But mail came Friday and I thought yes, it's here! But no
I have received a form from Universdal Credit 'Health Advisory Service' and I thought this was it - but I don't think it is? Form btm Right says UC50 12/20 and is entitled 'Cabability for Work Questionnaire'
This has confused me because I STILL have not received any further forms about PIP and this one is pretty long to fill-out too. But the letter with it states to supply 'copies of medical documents' to support what you say.. but when I asked my doctor about supporting documentation for the PIP forms, they told me they don't do that. I was advised to offer payment to the surgery on here but I don't really want to do that. So now I still have no PIP claim forms (the point-scoring forms as I received part one asking for Dr, bank info etc but that was all?) and now, I don't if you have any advice on filling this one in?
Any advice is vedry much appreciated. Also, could you juts send me a PS on how to search for my other posts?

You can click on your name in the sidebar of the post which is highlighted blue and it will take you to your previous posts.

The DWP have had IT forms which may have contributed to them being slow in sending out your PiP form, but I would ring and ask what has happened.
Supporting evidence can come from any medical professionals. It doesn't have to be the GP.
If you don't have any, you are entitled to access your GP or hospital medical records - though there's normally a charge for this.

There is no charge if you ask your doctor for a copy of your medical file under a subject access request, you have to put the request in writing. ico.org.uk/your-data-matters/your-right-...ject-access-request/

Hi Everyone,
I am happy to say that I finally received my PIP forms and have spent many hours following your guides. One Question:
I have been an illustrator all my life, my cervical spondylosis and other issues I now have with my hands, means it is much easier for me to type (even if its one key at a time) than it is to write with a pen.
Can I write several sheets with lists of the descriptors number (in order as they appear throughout the forms) and then my reply, simply listed over several pages and attach these pages via a staple to the front or back of the form (with my re Ni number and name at top of each page)?
Or..
Do I literally have to write a separate answer for each descriptor on a separate page and fix that page to where the answer should be in the box in pen?
I still have not been able to get any supporting evidence yet, I have even written to my old doctor (as he was brilliant) to see if he can help, but nothing back yet? Although gary in the last reply said I can ask my current doctor under a Subject Access Request (but I only saw this today!)
PS: Can I click a box somewhere to be notified when someone replies to a post here?

Hello Brian,
I frequently do what you describe in your first option. I tick the boxes on the form, and in the free text boxes on the form I just right ‘see separate sheet’. Make sure that each section is clearly labelled, and as you say include your name and NI number on each sheet. I also put a page count at the bottom each page 2 of 4, just in case something goes astray. You can staple them, but remember that the first thing that happens to your form is that it is scanned at Wolverhampton, so I put them in as separate sheets – I know Gary ties them together with treasury tags.
I have read of members doing your second option, but that sounds like hard work to me.
I have never found the click a box to be notified works, but no harm in trying.
If you are struggling to get evidence in time, you can submit your form as is, and then send in ‘additional evidence’ once you have received it.

I think I WILL write another letter to my current doctors and simply use the the tag mentioned in Gary's message and simply ask again? I guess the biggest issue is this: what on earth do you ask for specifically??
I have suffered with neck, wrist and hand issues for many years. I have had an operation to loosen the nerves/tendons in one arm to help the issues (in 2014 but it's worse now!) and I am also on antidepressants. I also have developed Dupuytren’s Contracture in both hands and having been an art teacher and illustrator all of my life, not being able to do this any more is mentally crushing
I have had MRI scans confirming the Cervical Spondylosis about 7 yrs ago and about 3 yrs ago X-Rays on my hip to confirm Arthritis. I get searing migraines and lots of pain and some days I cannot get out of bed at all.
My eldest daughter is a nurse in the Emergency Room. She has written a couple of A4 pages to support what I say (as she knows me better than anyone - and it does say you can ask family/friends) but with her being a qualified nurse, I am hoping that they MUST take this into consideration? She was there to hold my hand and assist with administering the anaesthetic when had the arm op.
Would appreciate any thoughts on this please?
