Hello Brian,
Ideally what you want is a letter saying that you have certain difficulties with everyday living and mobility, but the reality is that your GP is not likely to really know how your conditions impact on your daily life so this is not possible.
The next best thing is a kind of certified list of your conditions and the severity of them. This can be a combination of GP diagnosis, Consultant diagnosis, test results and medication and treatment.
The HP should then compare that list with list of difficulties which you have declared that you have and see if, in their professional judgement, the two match up. It is my impression of having read too many assessment reports that they are looking for consistency; consistency between what you say on your form and what you say in the assessment and, of importance here, consistency between what you claim and the evidence you offer.

Ok, I have spent SO many hours having to type both UC50 and PIP2 forms. I have said in my evidence that I must do it this way due to issues writing (suffer with my hands etc). I do hope this doesn't go against me! Both forms replies have many of my CBT Steps to Wellbeing counselling sessions which I received successfully via SARs. These sessions go back to 2012 and one of them has a letter dated from my doctor back then, saying i have suffered ling term depression. It also mentions the Ulna contraction (which i had to have an operation for in 2014) and several times in the notes written by the counsellors, it refers to my on-going chronic pain and how it is beginning to effect my work as a creative person.
This reads almost like a timeline of how the issues got progressively much worse and of course, covers my depressive & anxiety scores on some sessions as Severe. I can only pray that this is currently enough evidence because my drs are Still refusing to give me any medical records as they say they are ‘too busy’ Hmmm!
So, this is what I have done: I used to live up North and had doctors there for many years. The practice up there diagnosed my first hand and wrist issues in 1990. I was not able to work for 9 months because of this. I have written to them mentioning the SAR and the 1988 ruling and asked if they can assist with records?
I am also writing to the hospital to the hand, arm & wrist consultant who operated on my arm in 2014 for a letter of confirmation.
The forms will have to go tmorrow as the deadline is 6th Sept and if i am successful receiving any medical info, then I will send Recorded Delivery (and that is how I will post my forms too)
I wanted to say a MASSIVE THANK YOU as I spent days and days using your guides for UC50 and PIP2 and I wouldn't have been able to complete them so efficiently without them. I smiled when I got to the end and read the comments saying Congratulations! And how we hopefully will never have to do such firms again!
I just hope (as I said at the top) that my 47 pages of writing and copied evidence doesnt in some way back-fire!! I worked for weeks on these and I feel confident that apart from any doctors records (fingers crossed) that i quite simply have done EVERYTHING i could to make a damn good effort.
I will keep you All posted

Wow I am impressed Brian!
I agree with you, and hope all your work leads to the right awards. You say that the forms need to be back by the 6th. If you need an extension to this date you can ring up the DWP and request one, they are generally pretty helpful, but you can also submit your forms now and then send in additional evidence as you get it. As with everything you send to them, make sure that you have your name and NI number on each page.

Hi Brian, and so sorry to hear how your physical problems have crushed your creative career, this must be hard indeed. Just to add a small thing, that if expecting to send additional evidence later for a form I'm sending for a deadline, I would always note on the form that they should expect additional evidence I'm planning to send.
I just feel in the crazy workload of the DWP that this may improve the chances of the staff reading the form looking for the extras in their system and actually joining them up. Some people give it a day or two after the recorded delivery tracking says they have got it, then phone to check it has been joined up.

Hi Brian, don't hesitate to ask for an extension if you need one. I requested one and was given an extra 2 weeks plus told I could ring back and request an additional week for posting. It is generally much easier to get through on the pip helpline if you ring as close to their opening time (9am) as possible.
