PA4 Assessors Report / Mandatory Reconsideration Request

Dear DWP,

I am requesting a Mandatory Reconsideration Request for my PIP application (letter dated 8 Jan 2022.) I will be supplying more information with regard to this very soon but first, I am requesting the PA4 Assessors Report.

Please send the PA4 Assessors Report to me at your earliest convenience. After reading through the PA4 report I will then send details and more supporting evidence for a Mandatory Reconsideration Request.

Dear Brian, in addition to Gary's good advice, in your MR you can state prominently that you intend to submit additional evidence as soon as you are able to do so. This allows you to get the initial request in without being late for the one-month deadline, then send more after you have had time to receive and digest the PA4 report. Do be sure that all sheets have your name and NI No on the top, and that the second lot has a cover pointing out very prominently that it is to be added to the paperwork already sent. I suggest a phone call once the Post Office tracking confirms they have the second lot plus a day or two, to confirm they have put the two together.

OK, so just to clarify (as I do not have time to get this wrong as need to get this by 8th Feb deadline) can I send the letter as above text ONLY or - do I HAVE to add the reasons for my MR?

I don't want them cheekily telling me that this blanket statement was a reason for them to read thru again and come to same conclusion without any pointers!

Or - do I have to include the reasons in this (before deadline ends) with my pointers outlining their opinions of error?

As BIS has stated your text is fine, I would add that you disagree with the decision makers decision which is based on the assessors report.

Hi everyone,

FINALLY I have received the PIP Consultation Report Form PA4! This only happened bcos DWP called me last week and said they had noticed that I had asked (as they were about to look at my claim again via the Review), I had asked for this form and never been sent it. They offered to do it there and then (over phone) but I said no thank you, my advice was to receive this first. It then arrived yesterday..

So my friends.. what do we do next?
