Yes, I wrote this on the opening pages. I said I had asked referring to the SARs ruling and was waiting, but did not want to miss the deadlines. Having read these replies today I have decided to ask for an extension.

Yes, I have decided to ask for an extension. In some ways, I really would like to get them in the post and out of my site! But also, I did not realise I could ask for the extension and of course, as mentioned (and it was something I was concerned about tbh) I don't want it to get 'lost' amidst all their crazy workload. So I guess (if possible) it would always be better to send it ALL together.

Hi Brian, good luck with it all. If you ask for an extension you don't have to use all of it but it gives you a cushion to be sure you are happy with everything before sending it. I do understand what you mean about wanting to get it sent though.

Hi Brian
I believe that there is no extension available for completing the WCA form unlike a PIP2 form even if you are told that you can have one!
You can return the form Late but I don't advise this, it would be better if you return the form by the required date and then send any evidence that you are able to collect as soon as you get it. Take a note of the address on the envelope for the form and send them to this, send a copy to the DWP office dealing with your claim, both with a covering letter explaining what they are.

Hi All 
OK, I sent off all the forms to PIP & UC50. I managed to get a second set (under SAR's) of notes from Hospital so sent these on too (to both)
I have received a letter from UC 'Health Assessment Advisory Service', saying I have an Appointment for a telephone assessment with a healthcare professional' next week.
Could you pls give me some advice on how I need to conduct this call? It seems to me that a phone call is better than going in (so that's a bonus - I hope!?) but do they ask you ALL the SAME questions as were on the forms I filled-in?
Any advice most greatly appreciated.. wee bit closer now.

Thats the first stage completed so well done, did you send the forms off by recorded delivery?
If you have a look at our PIP guide at the link below you will see that there are 17 pages about the telephone assessment, starting at page 105.
A lot of the questions are those that would have been asked if you had a face to face assessment.

- Write yourself a bullet point list of any particular points you want to get over, so you have it by you during the assessment, tick them as you cover them during the assessment.
- have paper and pens to hand to make any notes such as the assessors name, date, time
- make sure you are seated in a comfortable chair, you are going to be there a while and at table to make notes
- if you can't hear or understand what is being asked then ask them to repeat the question
- don’t let the assessor rush you and get you flustered.
- don't forget to have a glass of water if needed so you don't get a dry throat and lose your voice!
Don't forget to contact DWP 48 hours after the assessment and ask for a copy of the assessors report called PA4.
