Yes just a list of dates. the NHS therapist curtaled the treatment when my daughter was referred to the mental health trust. The private counsellor would not give her a supporting letter.

Ideally you want an explicit statement that the report applies to the date of the Decision.

It is up to you on whether you think this information will help to show that she did suffer from limitations at the time of the adverse decision, but if you don't submit it, it cannot possibly be taken into account, can it ??? :)

Just an update. We obtained dates of GP consultations which have references to 'Anxiety and depression'. These documents predate the PIP application,by a few weeks, so proving that there is a link to later evidence. However, our representative seems to think this is not relevant and I find this surprising. We also await a SAR request from two previous conselling courses which I believe will be important, and I hope this arrives in good time to send them on to the tribunal.
