typiing getting bad,,,not used to being on line and have typed so muuch too hands etc in agony,, can hardly see out of my good eye andd thats kmade worse by mme being so disstressed and sobbbing

The whole form gives you the opportunity to paint a picture of how your medical conditions impact on your daily life. The people who read your completed questionnaire do not know anything about you so the more information you can give them, the better.
