Hi asc6670
I'm sorry to hear about your seizure.
As I've said before, we can't tell you how to answer any of the questions. You know your medical condition/s and the impact it has - on you and you need to explain it on the form. If you don't know whether to put, yes, no or sometimes then leave it blank but put an explanation in the box.
The only thing I can suggest is if you are too unwell to fill it in at all - you say so on the form and leave it blank. The only other thing I can think of (which you may not be able to do because of your agoraphobia is contact your local citizen's advice bureau and see if they would be able to help you, even over the telephone.
You can also contact the DWP and ask for a two week extension to returning the form.

Hello again asc6670. I too am sorry to hear about your seizure and hope you are recovering now. I recently had my ESA renewal, on top of my ‘Invitation’ to transfer from DLA to PIP, I also struggled with the handwriting and opted to do the ESA online which allowed me to save and edit as much as I liked. For some reason PIP application is not offered as an online option, I contacted the B&W team and they provided me with an email address to request an Alternative Format Questionnaire, which I’ve now done.
It will not help you at the moment, but I will report back here on what the process entails. Perhaps if you can contact the DWP and explain your situation they may offer you an extension, I find it difficult to understand why a claimant’s health situation is not taken into consideration given the nature of the benefit you are trying to claim. If you have had benefit before, did you receive help with that application? Is no one available to help you this time round? Sue

thank you both. x
oh dear sue, hell, am so so sorry and yes, i am now seriously worried sick i may too get a sudden dwp and a esa assessment too.
i find it hard to type, but better to write even thouggh its so much agony, not just painwise, but physically too, plus if youve got to give so much additional info that drags up the past traumas which again, makes you iller.
i do wish you luck sue [hug] and iif you find it better on line, then good.
if you mean did i have help when first applied for dla? yes, mum who was my carer and course several drs etc etc.
now i have lost my mum, i am lost. my drs two of them that knew my faamily too both took early retirement just after i lost my mum [lost my beloved lab sog 2 weeks befor xmas, lost mum 2 weeks later.
yes, my new dr and other drs at the practise are helpful, but they can only go on notes, they dont realy know me well iif that makes sense.- but in saying that they fully back mme and know all my problems, which would take 5 or more hours to explain.
there lies the issue, i suffer from so much, not just one thingg.
i am meant to go to pain clinic beginning nov, im already sick to the stomach about going or whether i'll even be able to go.
thats why me, the drs dont know how we are meant to reply to the questions 1 up to question 9
[as said, i sometimes go out once a year, thats it and when i do go out, yes, i need much help]
im so exhausted but will see if any where else by phone could helpp, im too frightened to ring dwp, phones make me panicc
and when youre alone, have no family left and friends have deserted yyou, just mmakes everything so much more frightening., sorry cant think of better word.
50 yrs old amd am sitting here sobbing
sorry, back to form filling. now i have to rely mainly on phoning organisations to help me fill out forms [even the samaritans, which help too]
