Hi

I see someone else has asked about typing up responses but there should be an online form available under The Equality Act (reasonable adjustments), shouldn't there? I know several people who would need this.

On another matter when sending in medical evidence how far back do you need to go? I was just going to include last 5 years but I have stuff going back 15 years or more. I wasn't intending to include everything. Plus, do I need to send copies of prescriptions and blood tests?

The primary issue is that all PIP2 forms have a barcode that is unique to the claimant and used in the tracking of the claim, this would not be available if the form was available for download.

As a general rule, evidence that is older than two years is going to carry little weight unless you can show that due to the nature of your conditions it is still applicable, the exception to this would be diagnosis letters.

Hi

On ESA form there's somewhere to put dates we can't do. But there's nothing on the PIP form...should I just write dates I can't do on the form? I also don't want them to phone me.....I don't answer phone to people I don't know so I want to include this somewhere.

You could put it on the form or enclose it on a seperate piece of paper.

There have been discussions on the forum concerning phone calls. Do they have your number? Have they rung before? You can state that you have no facility for receiving phone calls.

I don't know if they have my number. I have caller display so wouldn't answer to an unknown caller. I think they do have my phone number as they tried to ring my sister some years ago. But I have asked for things in writing so I will emphasise it on the form.
