There's no need for you to be hospitalised, if you need an extension then just ring the PIP claim line and they should grant one.

I am housebound and alone most of the time and the kindly bod at DWP seemed to think I may be able to get a home visit from CAB. Last time I checked there was a 3 month wait to get a 10 minute appointment at the centre where I live, so will have to forget that. But I was wondering if it is possible to type out answers (voice to text software) and then cut out and stick it on the appropriate page for each explanation box? Or is that not possible? I was actually cogitating getting the whole form scanned in at correct size then fill out with typing and print off.... surely the bar code will be fine reproduced from a good scan?
(I might add that aforementioned helpful bod also said as long as I don't leave it til the day it's due in, I can get a second extension if absolutely necessary. He said the reason not to leave it til the last minute is so they can inform the paying office(?) in good time)

A couple of members have asked for an been given a second extension, but I would be cautious of doing so if you can avoid it and it you do, you should certainly ring at least a week in advance of your needing it.

Previously for my DLA I added my own tick boxes and literally wrote all over the form wherever it was needed to better explain, including sideways on and around the corners. I don't know if that will be ok this time
