Where it states face to face does that mean mixing with one person only or can it be in a group situation one person at a time?

Both or either as fits your issues. Would more people escalate your reaction to the situation, perhaps you cannot cope talking to even one person, does the situation matter or the person you are talking to, would you react differently for someone you know versus someone you do not, do you see talking to your GP as a social event?

Does this question deal with cognitive issues only?

The DWP guidance suggests that it is not a physical activity, although I think you could argue that a physical issue that prevented to you making a payment, perhaps sensory or dexterity issues, which would require the assistance of another person and would be covered by (b) and (c).
